The first MMO I ever played was Horizons, where I played a Sslik (Lizardman) named Ssilmath. I just came up with the name on the spot, and it's stuck with me for years now.

My initials are PAE. I've always signed my e-mails with my initials in lower case only, so 'pae' it is. Strangely enough, when I sign my initials, they are in all caps.

It's amusing when people capitialize the 'P' but leave the other two in lower case. Like they think it's my nickname or something. :-p

Mine comes from 3 places... one of my favourite bands is behemoth, moths are my favourite insects and like a moth, I'm useless through the day and only come alive during the night

Mine is an army nickname, as I hung around with friends who were massively taller than me (and I am not short, bah).

Mine came from years ago when I made multiplayer characters for HL and Quake 2, I wanted something that described what I was doing.
Nothing I made had any real physical prescence, they were just head works, sculpts that existed as a file on a platter.
I shortened it to make it quicker to type.

Since I'd used it on one forum, I used it everywhere else and its kinda stuck.

Mine is a name I took on the field of battle from a mighty foe who was swathed in eldritch might and carried a sword forged in the pits of despair

Mine comes from the late 90's when RP Chat was popular. I played in a Star Wars room and I wanted to come up with a really original name for a Dark Jedi character. I was really into rock climbing and I was doing some writing at the time and had a Gaelic dictionary for reference with coming up with cool names. Creagan Tur means Stone Tower. I removed the extra vowel and came up with Cregan Tur O'Shea for the character's full name. Ever since then I've shortened it to just CreganTur for forum use, as none of them would allow the apostrophe. It's a handle I've been using for at least 12 years.

Mine comes in 2 parts Eldin is a RP character and is elven for 'silent one' - he was a mute half-elf, and Tux is the penguin that represents Linux my operating system of choice.
